As a recipient of 9D Care services you have right to

• To be treated as an individual and receive the right care for you that is respectful.

• You have the right to your privacy.

• You have the right to have the best service we can offer.

• You have the right to give us information in the way that you can.

• You have the right to choose our services or not to choose our services.

• You have the right to be consulted about decisions that affect you.

• You have the right to ask for information in a timely manner.

• You have the right to ask for another member of our staff to support you.

• You have the right to provide us Feedback, Compliments & Complaints

As a recipient of 9D Care services you have a responsibility to:

• Provide us with complete and accurate information

• Notify us of any changes to your circumstances that could affect our duty of care to you

• Be polite and respectful to the Carers, Volunteers and 9D Care staff whenever you interact with them.

• Let us know when you need a change and can’t make an appointment.

• To abide by the terms of the written service agreement.

• To acknowledge that your needs change and to talk and modify your services according to your needs.

• Give us enough notice for your appointment needs.

• Notify us of any changes to your situation, including change of contact details.

• Use prescription or medical devices for oneself only.

• Give us a honest feedback and let us know when we are doing a good job or could improve.

• Tell us if you feel you are at risk of harm of any kind.

• Discuss any particular cultural needs you would like us to be aware of.

• Make sure your home is safe for our care providers to work in. This includes ensuring we have enough space to use the proper equipment and ensuring that when we are not using our own equipment that the electrical devices or equipment provided by you to use (such as mops, buckets and vacuum cleaners) are in good working order.
