Start times

We understand that discharge referrals can be urgent. We respond promptly and will state clearly what can be arranged. Complex health needs, two-worker support, active overnight staffing, accommodation, equipment, training or funding gaps may require more time than the usual 48-72 hour commencement target. 9D Care cannot approve NDIS funding, guarantee accommodation or replace hospital clinical decision-making. A discharge should proceed only when the responsible teams are satisfied that the planned setting and supports are appropriate.

QUESTIONS & ANSWERS

Frequently asked questions

The participant, their authorised representative, a Support Coordinator, hospital professional, social worker or another authorised referral partner can contact us.

We can begin the capacity check, but the information needed for a safe service, appropriate funding and agreed responsibilities must be confirmed before support starts.

Useful information includes the expected discharge date, destination, NDIS plan and funding, required support hours and ratios, medication and clinical instructions, mobility or equipment needs, risks, behaviour support information and key contacts.

We can check suitable STA, MTA or supported-living options where relevant, but accommodation depends on funding, availability, accessibility, compatibility and safe staffing.

The responsible hospital team determines clinical discharge readiness, and the NDIA makes NDIS funding decisions. 9D Care confirms only the services and capacity that we can safely provide.

The agreed team can monitor how the transition is working, document concerns, communicate with authorised providers and adjust 9D Care services within available funding and safe service arrangements.
