Support Coordination helps a participant understand and use their NDIS plan, connect with services and build confidence coordinating their supports. 9D Care can assist with Level 2 Support Coordination and Level 3 Specialist Support Coordination, subject to the participant's funding and current team capacity.
Level 2 support can help a participant:
Understand the plan and funded supports
Identify and compare suitable providers
Coordinate services and communication
Monitor budgets, service gaps and progress
Prepare for plan reassessments and changes
Respond to service disruptions
Build skills to manage more of their supports over time
Level 3 may be suitable where the participant has highly complex circumstances or significant barriers affecting service delivery. Support may include:
Stabilising complex support environments
Coordinating multiple disability, health, housing and mainstream services
Responding to critical service breakdowns or risks
Developing practical plans to reduce barriers and improve continuity
Supporting complex hospital discharge or housing transitions
Helping the participant and support network establish more sustainable arrangements
Participants remain free to choose their providers. If 9D Care provides Support Coordination and another service, we explain the potential conflict, offer genuine alternatives and document the participant's informed choice. A participant does not have to use another 9D Care service.
Support may be delivered face to face or remotely across Victoria, subject to capacity. Remote services are also available in New South Wales, Queensland, South Australia and Western Australia.
